Is your save file clean or old , as you have played fnv in the past for sure so it could crash due to that.
Or some kind of tweaks of the past have left there remarks on your install and thats crashing it , use steam file checker to.

Normally a crash like that means that the game is missing a master or one of the main files are corrupt. I am assuming that you got the game through Steam. If that is the case, open the steam library, right click on the game name and select properties. Next click on local files and then "verify integrity of game cache". This will essentially go through your game directories and do a check against what steam says is supposed to be there. If anything is out of place, new copies of the files will be downloaded. 90%+ of the time this will resolve these kind of issues.
If that doesn't work then the save is corrupt and you may need to go back to an earlier save (or restart the game..)..
One thing too... If you have more than 100 or so saved games the system can become a bit more unstable. I would suggest that you keep the directory trimmed down and if you want to save the older saves, move them to an alternative directory. Also rather than use the quicksave, make a regular save. Its safer overall...

@lukaboot It is a brand new save game, also brand new install as I got a new computer
@rgordonjr Validated steam files, forgot to mention that. Also I have no other save games on this, this happens when loading into a new game.
Thank you too

I remember having a shit storm every time I installed this game. I still have to keep an old save from after I exit Doc Mitchell's place because it CTD's when I exit for the first time.
Any third party gaming software running in the background?

Hmm... How are you launching the game? If you use MO or NMM make sure that the game files are checked and activated. I've run into problems in the past on my own game where the main FONV file was unselected...

@rgordonjr Launching through steam, then 4gb, then 4gb in admin mode, then 4gb in compatibility through all OS's.

@lukaboot - Just asking as I know several people who run the game via a mod manager even when playing unmodded.... (I also do that when I am starting a new mod building profile with MO)..
@gav07Gaming - At this point all I can really suggest then is going with the nuclear option... removing all game files via steam, then going in and making sure the steam directory was deleted as well as the save game directory... After that a full reinstall.
Good luck.

Nope. Literally no mods now. It is a brand new computer with a fresh install. When it wouldn't work I installed NVAC, NVSE and 4Gb but they have been removed @mpaz96
