Being a mod author mainly, designing Project Lore is fun and it keeps the game all Realistic, harder, and interesting. Being a mod author keeps me busy and it also helps me enjoy Fallout, quite sad to say for me if there was no mods for fallout i would have quit playing FO3 and FNV a long time a go, so really just download some mods of your choice, If you like realism mods go download some other mods...well ETC.

I like to relate my fallout experience to certain tv shows and movies I might be watching at the time, and adjust my mods accordingly, that's how I do it... sometimes I make it , book of elish, sometimes its the walking dead or other times its a real bright type game like battlefield

What really keeps me interested is usually the role play, I try to think as my character would in situations what are his/her goals?, is my character a smooth talker or does he let his guns do the talking? What really makes it interesting is what bonds you have with your character and your companions and all the memorable moments you create. Well at least this is what keeps it interesting for me, I don't really think games like fallout and tes should be played as just video games but more like you playing out a story in your characters life.

Well if you give Boxcars a Med-X for his broken legs you will gain karma, after that if you shot him(after completing his quest) you will gain even more karma

Its like one of those things with call of duty. There was that whole argument over weather or not world at war was COD 5, and people were doing that with GTA SA, saying it was gta 4 ect.

Have to say Hardcore mods along with increased spawns and that commander mod. Always fun to lose an entire ten man squad to a super mutant ambush, I mean, you could call down airstrikes- but what's the fun in that?

New mods and new quests, taking a different approach towards the game. Sometimes i skip the main quest at first and just immediately start doing mod quests. My favorite thing to do though is to like narrate in my mind what my character is thinking and focus on developing my character as i play, almost like writing a story.

Honestly, the best approach to this in my opinion. Is keep making new games, don't just progress through the entire thing once.
Make an NCR file, become their hero.
Make a Legion File, become the destroyer of the wasteland.
Make a neutral file, rule everything.
Make a Listen to every word house says file, become the lapdog of the wasteland... I mean lacky...
Do a file specifically for Melee, do one just with pistols, why not one just as a stealth sniper.
Those don't sound all that appealing? Well, heck just do your job as a courier if you want, forget the story, buy things on one side of the map, walk all the way to the other without fast travel on and sell them. Kill things along the way.
The wonderful thing about New Vegas, is no matter how many times you've 'beaten' it, there's always another approach. Heck, one time I decided to do an entire hardcore play through with just brass knuckles. It's Fallout, if all else fails grab a Fat Man and Nuke... everything. Since you know, New Vegas was missed, you could fix that problem.

To keep my game interesting I try to pick or make a few companions that I truly enjoy, and place them in hard to reach places, basically make my own quest to find them, and at the same time try to do different faction playthroughs, including one where I don't really side with anyone and I make up my own story with each of my team members

Start a new game, create a character, customize his skills/traits, give him a personality, play as him for some time, record everything for fun and just in case, install as many game mods as possible, save/load to see all possible outcomes of your actions, rince and repeat. And that's why I never finished neither Fallout 3 nor Fallout New Vegas.
