Is it possible your speaker settings got changed when you upgraded? (i.e. random unwanted enhancements got checked?)

Isn't there an option on settings to control volume options?

-Right click the speaker icon. -Right click your device. -Click properties. -Check the Enhancements tab. (and while you're in there check Levels and Advanced too)

On July 28, Microsoft released an update to the Realtek High Definition Audio Driver.
To get this driver you must be using Internet Explorer 8 or higher. You won't be able to download with any other browser that I'm aware of.
Realtek High Definition Audio Driver - July 28

Fallout has always been REALLY finnicky about realtek drivers. There were some posts about fixing it by changing surround sound to 5.1, and there were others that said you could fix it by installing K-Lite codecs. Personally (I also have Realtek) the problems just seem to come and go, even when you find a solution.

Drivers, codex, or sound settings are the three most common issues for this. When I experienced various sound issues with Fallout games ( specifically FO3) it was the condex that was giving the problems.
Most here have suggested various solutions from above. The best you can do since Window 10 has just been released is check to see if there are any new updates with your sound drivers through Windows Update. Then if that don't work try a few compatible codex or setting changes. Unfortunately there is little we can do for a quick fix of your problem because it can be many things.
Can you back up your computer and roll back to 8.1 or 7 Os that was working? At least for a month or so until Windows 10 had some time to be on the market to help resolve various issues?
Best of luck.

Have you tried "Loudness Equalization" in Enhancements? It may make everything fairlly loud but that should be fixable through master volume.
There are a few discussions on other sites about different issues with similar solutions. I'd recommend just googling "new vegas sound issues realtek".

right click the speaker icon, click playback devices, right click "speakers" go to properties, then go to enhancements tab. There you can see "loudness equalization" try turning it on or off see if it works

Thank you Sir or Madam for showing me something I never new existed.
Sorry that it didn't work for the OP.
A bit risky but have you tried to disable the sound drives and / or delete them and force an update for these drivers?

HEH HEH I see what you did there.
Glad to be of help. Turning that option off or on fixes my audio problems with volume personally. Too bad it didn't fix OP's problem though.

I'm a bit nervous doing that
Might be time to have someone that is trained on this to take a look. Before you do that make sure you have everything backed up just in case. Might be something so simple it is crazy but I don't see much more to do without getting your hands dirty and really tearing into the system and changing codex, disabling and renabling or even deleting and reinstalling drivers. It is messy and risky that you might have to reinstall.
Which brings me to the next thing. Have you tried reinstalling it. I don't remember if you stated you upgraded or clean installed. Sometimes upgrades aren't always the most stable and successful. Better than in the past but not as good as say Mac or Linux upgrading OS.
