Story we HEARD about Lanius have no proof of its accuracy.
It can't be accurate since he still has his face(it is possible
that is because game limitation but they made special faces for
some NPCs in that game) and they allow you to remove his mask,
but don't allow to remove his armor which is weird its like they
deliberately wants you to take his mask off.To further prove my theory
Ulysses points out that Lanius might be just a legacy character. When
the old one dies a new one takes his place, and Lucius mentions that Lanius
was already a legionary by the age of 12 wich contradict Caesar's story about
him.I think that Caesar made up story because legion needed a ferocious murderbeast to keep men in line and to scare NCR.
Those are just my thoughts what do you think about Lanius true identity.

The Legion also mentions that he murdered his tribe as an adult because they were "too weak" yet as you stated, at the age of 12 he was a Legionary and prior to that his clan was assimilated into Caesar's Legion. The story doesn't add up and isn't 100% accurate, but just like any legend there tends to be some truths that are stretched beyond fact.

The ancient Persians kept a standing army of 10000 soldiers called "Immortals", when every injured or killed member was immediately replaced, thus creating an illusion of being inexhaustible. The Legate wouldn't be the first person in fiction to be known more by his reputation than his face. The Dread Pirate Roberts springs to mind as well.

Personally, I think it is one man. A man who, above all others, thrived and grew in the fires of battle and the utter hellishness of the wasteland, becoming an unstoppable weapon. The Courier is similar in ways: survivor, warrior and master of the wasteland. I think it was a very appropriate moment in the story that these two characters meet.
