For realism: - Realistic Reloading (nexus) - Realistic Damages (nexus)
For more locations to explore: - Project Brazil (moddb) - Tales of Two Wastelands if you have Fallout 3 (taleoftwowastelands website) - A World of Pain (nexus)
For the better random encounters: DFB - Random Encounters perhaps? (nexus)
For the guns, I would suggest you Weapons of the New Millenia on the Nexus and aarius big weapon pack which can be found on this site.

if going for a hardcore play and you are using TTW i would suggest TTW NPC's use ammo nothing feels more hard core then getting into a fire fight and at the end finding almost no ammo cause they used it all.

Project Nevada adds a variety of functions like a grenade hotkey for seemess combat, toggles for food healing, damage from weapons both for and by the player and most importantly, visor overlays. Such a small thing, yet I can't wear helmets in this game anymore without the overlay, it feels weird.

I'm using a lot of mods. But for that certain realism we are speaking here, I have Project Nevada and on top of that Realistic Weapon Damages, BLEED and Unforgiving Combat. With these mods you can kill an enemy much faster, but you also die a lot easier. Head shots are fatal.
It's fun and makes the game more challenging without unnecessary bullet sponges.

@"IRORIEH" I have to be one of the only people that doesn't really like Project Nevada :/ I find the Visor overlays gaudy and distracting, the 'Rebalance' module is ridiculous for a game, it's one hit kill everything regardless of armour type or strength, the Night vision stuff is OP as fuck and the sprint is done better in the Animation Project. I downloaded it at the start of my modding experience with NV because everyone was raving about it and I gotta say, it did look pretty cool. But as time went on I found myself gradually turning off more and more of the features until the only thing left was the Grenade Hotkey For rebalancing an a whole lot more, I would always reccomend the JSawyer Ultimate Edition, not Project Nevada, but that's just my opinion

After War Nevada is an overhaul that I personally adore. It's basically FWE - FO3 Wanderers Edition designed for New Vegas, even more so than Project Nevada.

I still say this is the best realism mod out there, you need bandages, get broken bones, trauma, etc

You can deactivate everything or almost all of that stuff.
Animation Project is recent by the way, Project Nevada is pretty old by now. You can use them both at the same time, no compatibility problems.
Just saying that you can always play with the settings so they can be to your liking.

I love Wanderers Edition for Fallout 3, it's basically essential for my gameplays.
Checked After War Nevada and it looks pretty good mod and similar to FWE, but because of its incompatibility with other mods I don't see myself using it. Though it would be fun to even try it once. I have Project Nevada and I'm happy with it.

Project Nevada allows you to customize the game however realistic you'd like it to be. So as far as combat I always enjoyed realistic damages as well.

Hmmmm...BLEED and Animation Project by RoyBatty, Realistic Bullet Speed GRA, Saints and Sinners, Project Nevada, Helpless Falling.
I can't really name them all since there's so many, but here are my top picks for what I think are good realism mods. Oh I almost forgot about Real Recoil.

For more weapons I'm using the great R.S.A. mod from this website: It's a russian site, just use Google Translator (worked for me to find the dl link) Other than that I can recommend using the mods Xilandro uses in his streams, it's a great list of pretty much the most realistic mods for the game which works well (I'm using more than the half of the list). Link to his Twitch Page, most of the mods have direct links to the corresponding site

I suppose it's just how you like to play the game. I always liked the ability to toggle things like food healing off, the amount of damage you recieve and that sort of thing. I can see how the visor overlays could be annoying, but on some of the darker nights in my game, the night vision feature is a necessity.
