1. We're talking about which Fallout game?
2. Post your load order if you're using any mods?
3. Post your PC specs?

And which fallout are you talking about..? fo3 new vegas or fo4..?

decrease the Lod distance lower other graphical settings like shadows or textures or if this did not solve your problem decrease the game resolution

If you are using ENB recommend to disable shading, depth of field and ambient oclusion. I do not know how to help you, even if not exactly have described the specifications of your computer and what you use in your game.

That's just it, you have provided enough information that anyone could help you put your load order and mods you're using and what hardware you're playing.

You can always check YouTube. There's an ultimate performance video with a couple of mode that disables tiny objects in game and thus freeing up a bit of fps.
You can try the new Vegas 4gb patch. It makes fnv use 4gb of memory instead of 2. You can use enb boost without the graphics but just the memory expanding. Had soms problems with fnv myself in the past and these helped. do you have an amd or an nvidia GPU?
